2022 Camper Applications Are Closed

There is a $50 camper fee per camper participating. 

This will not be collected until after your camper is accepted through the application process.

The application will include an interview for the Camper & Caregiver to meet with someone from our leadership team. 

Step One: Initial Application & Schedule Interview

After you complete the application, you will receive a link to sign up for a zoom interview, both Caregiver & Camper need to be present for the zoom interview.

Step Three: Prepare Information for Medication Administration Forms If Applicable

All medications need to have a Medication Administration Form properly filled out. Each medication must be sent in it's original container - Only send the exact number of pills for 7 days. We ask for the number of days at camp plus 2 extra days worth.
You will need the information for this while you are completing the disclosure forms in the next step. Here is the information that you will need to have prepared: Medication, Dosage, Frequency, Reason for Medication, Schedule for Adinistration (breakfast, lunch, dinner, as needed etc) & Side Effects or Any Specific Instructions

Step Four: Pay Camp Application Fee & Complete Online Disclosure Forms

Online Disclosure forms are completed through the website of the facility we are going to. You will receive detailed instructions with the login & password to access these online disclosure forms. The forms include: Camper Medical Form,  Colorado Immunization Form,  Horse Riding Release Form,  Activity Release Form,  Covid Release Form,  Kings Team Camp Release, Photo Release & Most Recent Physical 
While completing these forms, there will be an option to upload the copy of your most recent physical (within the last 12 months). If you need a blank form for teen's doctor to sign, you can use our optional physical form below. If you have trouble uploading the physical, you can email it to us at

Note if you did disclose medications your teen will be bringing with them to camp, you will receive a follow up call to verify we have all of the info submitted correctly for our Medication Administration Form. You will also receive a call a few days prior to camp to confirm nothing has changed in medication since completing these documents.

x Please do not bring clothing with questionable pictures or wording. 


x No candy, electronic games, alcohol or illegal drugs. 


x Please pack belongings in a duffel bag, suitcase, or a heavy-duty trash bag. 


x Sleeping bag without a cover bag must be packed in a heavy duty trash bag. 

x Be sure all your items are clearly marked with the camper's name.
