2024 Camper Applications

The application will include an interview for the Camper & Caregiver to meet with someone from our leadership team. 

There is a $50 camper fee per camper participating, which will be collected after your camper has completed their interview.

You will need to upload copy of your teen' most recent physical. The physical should be completed between 7/15/22-Presnt. If you need a blank form for teen's doctor to sign, you can use our physical form below. You can upload this form inside of your initial application OR you can use the upload link below. 

We also need a copy of your teen's current imunizations. Instead of completing it on the application, you can submit a copy of their immunization record from school. You can upload this inside of your initial applicaiton OR you can use the upload link below.

If medications change prior your teen attending camp, you must complete an updated Medication Form.
